This is the "Core Question" form. It is called from Template:Discipline to add new entries stating that such and such a question is a core question of the discipline. When the form is used to create a new core question claim (as opposed to merely editing an existing one), it expects the following parameter to be passed onto the query string:
query string= Discipline= //the FULLPAGENAME of discipline
